Mountain Valley Hospice Is currently accepting applications for the following positions FULL-TIME INTAKE/TEAM LEADER REGISTERED NURSE Responsibilities Include:
intake of referrals, answering inquiries, collaboration with community agencies.
& daily guidance to the Hospice team staff.
Must demonstrate outstanding critical thinking skills, communication, organizational and multi-tasking abilities.
Hospice experience required.
Benefits include competitive salary and benefits including paid time off PART-TIME QAPI COORDINATOR Position will coordinate the Quality Assessment Performance Improvement activities under the supervision of The Director of Clinical Services JOB DUTIES (including but not limited to) Utilizes internal data bases and comparative data systems to assist DCS with monitoring of organizational Performance:
clinical, finance, best practice.
Creates and implements Performance Improvement Projects, when required; Educates staff on needed improvements based on data and statistics; Plans and facilitates QAPI meetings Hospice Experience preferred Computer proficiency required FULL-TIME HOSPICE RN CASE MANAGER NYS RN License, 2 years' experience in an acute care setting.
Previous Hospice, homecare and/or oncology experience As a member of the Interdisciplinary Team, you will receive personal satisfaction knowing you make a difference in the lives of patients and families faced with a terminal illness.
Travel and excellent driving record required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
